How Our CARE PROGRAMs Work

Bringing In-Person & Virtual Care Right to Your Residence

We offer two programs designed to support you. Both programs provide year-round access to primary and urgent healthcare solutions, so you can receive personalized, high-quality care when you need it.

Step 1: Getting Started

After signing up, all Perennial Program participants will receive an in-person visit from their dedicated Nurse Practitioner, who will introduce themselves and begin your onboarding process. During this visit, they will also schedule your onboarding call, where our team will guide you through the program, explain our personalized approach to your care, and address any questions you may have.

Step 2: Your Baseline Visit

Your Baseline Visit is a crucial step in your health journey, allowing us to establish a comprehensive understanding of your current health status. During this visit, your dedicated Nurse Practitioner will conduct a thorough assessment, this visit serves as the foundation and ensures that all future care is tailored to your unique needs.

Step 3: Access Year-Round Care

Enjoy unlimited in-person and virtual appointments for primary care, urgent care, check-ups, specialist referrals, prescription renewals, and more — any time you need care. Book in-person or virtual appointments anytime — online, by phone, text, email, or in person.

Step 4: Access On-Site Healthcare Services

Every month, our network of healthcare providers and specialists offer convenient, on-site health services and treatments right at your residence. Your Perennial Care Team will inform you ahead of time and can help coordinate everything prior to your appointment.

Your Dedicated Physician

Your Physician is available to provide more advanced medical care and oversight when you need it most. Your Physician collaborates closely with your Nurse Practitioner and Care Coordinator to streamline treatments, arrange specialist referrals, and keep your overall healthcare journey on track.

What They Can Help With

  • Comprehensive Examinations: Regular physical exams, follow-up on chronic conditions (e.g., diabetes, hypertension).

  • Complex Medical Concerns: Assessing more serious symptoms, ordering advanced diagnostics, or managing multifaceted treatment plans.

  • Specialist Referrals: Directing you to additional specialized care (e.g., cardiology, orthopedics) if needed.

When You Can Access Them

  • For same-day or next-day appointments if a higher level of medical expertise is necessary.

  • When your Nurse Practitioner recommends a physician consult or if your condition is more complex.

Your Dedicated Nurse Practitioner

Your Nurse Practitioner is your first point of contact for day-to-day medical needs. Whether you have a sudden concern, need a quick check-up, or require a prescription refill, your NP ensures you receive prompt, professional care so you can feel confident knowing you’re well-supported at all times.

What They Can Help With

  • Illnesses & Injuries: Colds, flus, sore throats, minor sprains, or suspected infections.

  • Medication Management: Prescription refills, adjustments, or over-the-counter recommendations.

  • Routine Check-Ups: Blood pressure checks, basic screenings, and preventive care advice.

When You Can Access Them

  • For same-day or next-day concerns that need prompt attention but may not require a physician’s involvement.

  • Anytime you want a professional medical opinion quickly, whether virtually or on-site.

Your Dedicated Care Coordinator

Your Care Coordinator acts as your personal “healthcare quarterback.” They’ll arrange for any type of care visit or specialist visit. Your Care Coordinator will also work with you to leverage your existing insurance or benefits when accessing on-site services by specialists or other healthcare professionals.

What They Can Help With

  • Scheduling & Coordination: Booking appointments with the NP or other services.

  • Benefit & Billing Assistance: Helping you understand coverage and handle direct billing when possible.

  • Healthcare Navigation: Offering guidance on which provider might best meet your current needs.

  • Anytime you have a question about “who do I see for this?” or need help booking your next appointment.

  • When you want to make multiple appointments or coordinate care between different providers.

How do I sign up for Perennial?

You can sign up by visiting the Perennial Care Station in your residence. Our Care Coordinator can also walk you through the process in person or over the phone.

Do I need special insurance to join?

No, you don’t need special insurance. Our program works alongside your existing health or benefits plan. Most residents already have benefits they can use to cover certain services. Our team helps with billing to minimize any out-of-pocket costs.

What if I already have a primary physician outside of Perennial?

You’re welcome to keep your existing physician. Your Perennial care team will coordinate with your external doctor to ensure seamless communication and continuity of care. You’ll still benefit from on-site care, virtual appointments, and immediate access to our specialists.

Can my family or caregiver be involved in my care plan?

Absolutely. With your permission, we can share updates, coordinate appointments, and discuss care plans with family members or caregivers so everyone stays informed about your health journey.

What if I experience a medical emergency?

Perennial provides same-day or next-day medical care for routine and urgent needs. However, in a life-threatening emergency, call 911 or your local emergency number immediately. Our service is designed to supplement (not replace) emergency care.

Which program is right for me — Perennial Care or Perennial Premier Care?

  • Perennial Care is perfect if you need convenient, day-to-day care.

  • Perennial Premier Care is ideal for those who want comprehensive, proactive, physician-led care, including advanced diagnostics.

What if I need a service not listed (e.g., dental or specialized therapies)?

We’re continually adding specialists to our  On-Site Concierge Health Services. If a service isn’t currently available, talk to your Care Coordinator — they can often arrange a visit for any type of care specialist.

How do I book an appointment with a specialist?

Your Concierge Care Coordinator will handle the scheduling for any on-site specialist. You can also book telehealth appointments through our online portal or by phone.

When will Perennial be available in my area?

We’re rolling out Perennial to multiple residences across Canada. Check with your local residence or our website to confirm if Perennial is available where you live.
