Understanding The Role of Your Care Team

Physician

Our Physicians are available to provide more advanced medical care and oversight when you need it most.

What They Can Help With

  • Examinations: Regular physical exams, follow-up on chronic conditions.

  • Complex Medical Concerns: Assessing more serious symptoms or managing multifaceted treatment plans.

  • Specialist Referrals: Directing you to additional specialized care if needed.

  • For homes where Physicians are part of the care plan and your needs may be more complex.

  • When your Nurse Practitioner recommends a physician consult or if your condition is more complex.

Nurse Practitioner

Our Nurse Practitioners are your first point of contact for day-to-day medical needs.

What They Can Help With

  • Illnesses & Injuries: Colds, sore throats, minor sprains, or suspected infections.

  • Medication Management: Prescription refills, adjustments, or over-the-counter recommendations.

  • Routine Check-Ups: Blood pressure checks, basic screenings, and care advice.

When You Access Them

  • For primary or urgent care concerns that need prompt attention.

  • Anytime you want a professional medical opinion quickly, whether virtually or on-site.

Care Coordinator

Our Care Coordinator acts as your personal “healthcare quarterback" for all your care needs.

What They Can Help With

  • Scheduling & Coordination: Booking appointments with the NP or other services.

  • Benefit & Billing Assistance: Helping you understand coverage and handle direct billing when possible.

  • Healthcare Navigation: Offering guidance on what might meet your needs.

When You Access Them

  • Anytime you have a question about “who do I see for this?” or need help booking.

  • When you want to make multiple appointments or coordinate care between different providers.

What if I have a primary physician outside of Perennial?

You’re welcome to keep your existing physician. Your Perennial care team will coordinate with your external doctor to ensure seamless communication and continuity of care. You’ll still benefit from on-site care, virtual appointments, and immediate access to our specialists.

Can my family or caregiver be involved in my care plan?

Absolutely. With your permission, we can share updates, coordinate appointments, and discuss care plans with family members or caregivers so everyone stays informed about your health journey.

What if I experience a medical emergency?

Perennial provides same-day or next-day medical care for routine and urgent needs. However, in a life-threatening emergency, call 911 or your local emergency number immediately. Our service is designed to supplement (not replace) emergency care.

What if I need an allied health service not listed?

We’re continually adding specialists to our  On-Site Concierge Health Services. If a service isn’t currently available, talk to your Care Coordinator they can often arrange a visit for any type of care specialist.

How do I book an appointment?

You can also book appointments through our online portal, by phone or by contacting your Care Coordinator.

When will Perennial be available in my area?

We’re rolling out Perennial to multiple residences across Canada. Check with your local residence or our website to confirm if Perennial is available where you live.
